To Register for a Cheer and Dance Extreme competition event you must submit:

1. Registration Form (One per organization/gym or studio)
2. Team Roster (One for EACH Team for cheer and performance number for dance)
3. Medical Waiver/Media Release (One for EACH participant and EACH coach - valid 09/01/07 - 08/31/08)
4. Payment in Full (If paying by credit card you must include CDE's credit card authorization form)
